Myself or my husband having been sick in 3 third world countries (and without insurance) we had no problem getting care- for under $20. My first time was in China, I had a bad cold and no insurance. The hotel had me speak with a doctor and I was sent off with antibiotics- total cost $12. In India, I had another infection. Antibiotics and over the phone diagnosis of $2. In Bali, my husband had a fever- pills and a trip to the hospital $20.
My favorite though was Germany where I lived as an exchange student. The towns doctor came to our house and diagnosed me in my room.
